Camp Day Permission Slip

Your child has the opportunity to participate in a class field trip. Listed below is the information. Please make sure that you fill in all the blanks and sign the form by August 26th.

Tri-State Bible Camp & Conference, Montague, NJ
August 29, 2024

8:30 a.m-9:00 p.m.

Approximately 68 students will be attending the trip with 9 school staff members and personnel chaperoning the event.

Transportation will be provided by our bus and two school vans
