Parent/Student Agreement for Athletics

Canaan Christian Academy will provide a Christian environment and positive atmosphere for students to participate in.  Parents and coaches must cooperate to maintain this atmosphere.  Students must commit to be a faithful and diligent team member.  The following are specifics the need to be adhered to be a student-athlete at Canaan Christian Academy:

• Students must have a sports physical for the current year and the form must be completed by the doctor and parents before a student may practice.
• Parents must pay the athletic fee for the season before a student may participate.
• Students must attend all practices and games.  If a student becomes ill, he/she must communicate with the coach that he/she will not be at practice.
• If a student is absent from a practice or game, he/she will not be able to start in the next game.
• Students must ride to the games with the team on the bus or vans.  They may ride home with their parent after the game.
• With any sport there is an assumed risk for injury.  Parents must have medical insurance on their child to participate.  Parents must supply a copy of the medical card to the school office.
• Parents and students must realize when one joins a team, they are committing to practices, games, and the team for the entire season.

We have read the Athletic Handbook and agree to the above-mentioned items.
