MODEL RELEASE for minors

Throughout the school year Canaan Christian Academy will be taking numerous photos of our students at various events and activities. These photos are used for our school newsletter, website, our Facebook page, Instagram, and displays on Sundays in slide show format in Canaan Bible Chapel. We will not use your child’s name on any picture used on our school website, Facebook, or Instagram.  These media are for the enjoyment of our students and family and a way to keep our congregation and other supporters updated on all that we are doing. On occasion we will purchase advertising in the local media and use a photo or two. 

We want to assure parents that we take every precaution to protect the privacy of our students and staff. Photos on our website are such that they cannot easily be copied and enlarged and be recognizable. Our printed material is done by a Christian business who understands the need for sensitivity in this area as well.  Please read and sign the following.  If you have any questions, please contact Mr. Michael Boole.  This release will remain in your child’s permanent record until such time as they withdraw or graduate from the Academy.  If at any time, you would like to withdraw your consent as stated herein, it will be your responsibility to contact the school office and advise them of the same.

MODEL RELEASE FOR MINORS

I hereby grant permission for Canaan Christian Academy to use photographs and/or video of my child(ren) as described above. Furthermore, I permit CCA:

a) To copyright the same in Canaan Christian Academy’s name or any other name that it may select;

b) To use, re-use, publish and re-publish the same in whole or in part, separately or in conjunction with other photographs, in any medium now or hereafter known, and for any purpose whatsoever, including promotional material, website, internal productions and any other printed material that is solely for the purpose of illustrating life at CCA.

c) To use my name or my child’s name in connection with the photos (not those used on the school website, our Facebook page, or Instagram) if it so decides.

I hereby release and discharge Canaan Christian Academy from all and any claims and demands ensuing from or in connection with the use of the photographs, including any and all claims for libel and invasion of privacy.

This authorization and release shall inure to the benefit of the legal representatives, licensees and assigns of photographer as well as the person(s) for whom he/she took the photographs.
